APPOINTMENTS are made by calling the office at (419) 865-7500 between the hours of 9:00 a.m. - 5:00 p.m., Monday thru Thursday, and 9:00 a.m. - 4:00 p.m. on Friday. Special arrangements may be made for other appointment times
